The Bosniak classification consists of a number of different entities including septa characteristics and enhancement, potentially resulting in differences of interpretation among readers [1–3]. In order to validate the classification system further, studies regarding intra- and interobserver variation after the introduction of the updated version of the Bosniak classification are warranted. Based on Cohen's weighted kappa statistic (k), inter-observer agreement was moderate for CEUS (k=0.49) and fair for CECT (k=0.36). Agreement between CEUS and CECT for both readers was fair (reader 1, k=0.24; reader 2, k=0.37). Historic points In 1983, using early computed tomography (CT) scan technology, renal cysts were discovered in 33% of patients in the same age group. It was devised by Dr. Morton A. Bosniak to separate lesions that require surgery (Bosniak III and IV) from those that can be safely followed-up or left alone. Category Description Bosniak I Benign simple cyst with thin wall without septa, calcifications, or solid components. In 1986 Morton Bosniak published a review article in which he suggested a classification and further management of cystic lesions of the kidneys based on findings on contrast-enhanced computed tomography (CT) (1). The classification was gradually adopted by imaging specialists and urologists, and is currently a reference in the field (Figure 1).
